{"id":1318,"date":"2017-08-18T16:51:37","date_gmt":"2017-08-18T16:51:37","guid":{"rendered":"http:\/\/localhost:8080\/mywordpress\/centers-of-excellence-requirements\/"},"modified":"2020-01-15T09:17:53","modified_gmt":"2020-01-15T17:17:53","slug":"coe_reqs","status":"publish","type":"page","link":"http:\/\/142.11.238.239\/es\/coe_reqs\/","title":{"rendered":"REQUISITOS DE LOS CENTROS DE\nEXCELENCIA"},"content":{"rendered":"<div class=\"l3-header-con no-banner\"><span data-mce-type=\"bookmark\" style=\"display: inline-block; width: 0px; overflow: hidden; line-height: 0;\" class=\"mce_SELRES_start\"><\/span><\/p>\n<div class=\"l3-header-icon\"><img decoding=\"async\" src=\"\/wp-content\/uploads\/2017\/08\/COE_1.jpg\" alt=\"products\" \/><\/div>\n<div class=\"l3-header-text no-banner\">REQUISITOS DE LOS CENTROS DE\nEXCELENCIA<\/div>\n<\/div>\n<div style=\"clear: both;\"><\/div>\n<h3 class=\"bold_padding benefits\">Requirements for Consideration<\/h3>\n<p>Centers of Excellence must have well-established tracheostomy teams, <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span> Policies and Procedures, and must have been using the Valve independently in clinical practice for greater than three years. <\/p>\n<ul>\n<li>An identified <em>multidisciplinary team<\/em> for tracheostomy care and <span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span><sup>\u00ae<\/sup> Valve application, including physicians (e.g. intensivist, pulmonologist, otolaryngologist, pediatrician, neonatologist) nurse practitioners, nurses, respiratory therapists, speech-language pathologists, and other clinical staff, as appropriate.<\/li>\n<ul>\n<li>A multidisciplinary team for patients with tracheostomy established for a minimum of 3 years.<\/li>\n<li>Appropriate documentation on each team member to illustrate their qualifications and contributions to the team.<\/li>\n<\/ul>\n<li>Utilization of the <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span> in-line with the ventilator, when these patients are available.<\/li>\n<li>Written policies and procedures regarding <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span> use.<\/li>\n<li>Team and clinical staff participation in <span class=\"pm_ws\">Passy Muir<\/span> educational opportunities (e.g. in-services, webinars, self-study CEUs, etc.).<\/li>\n<ul>\n<li>An ongoing method for staff education and training regarding the assessment and application of the Valve (e.g. yearly competency, training checklist, supervised placements on mannequins and patients, in-service, webinar, etc.).<\/li>\n<li>Documentation of the education and training in which each team member and other clinical staff have participated.<\/li>\n<\/ul>\n<\/ul>\n<h3 class=\"bold_padding benefits\">Application Process<\/h3>\n<p>The rigorous application process to become a Center of Excellence involves a multi-level review process. The applying facility must submit documents to support the following phases and to confirm the practice of and commitment to a high standard of patient care:<\/p>\n<h3 class=\"bold_padding benefits\">Phase One:<\/h3>\n<ul>\n<li>Online application (via jot form).<\/li>\n<li>Facility\u2019s <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span> Policies and Procedures. <\/li>\n<li>Curricula Vitae or resumes and a 50-word bio for each member of the facility\u2019s multidisciplinary tracheostomy team, including their disciplines, related competencies, and roles on the team.<\/li>\n<li>Facility\u2019s philosophy of care.<\/li>\n<\/ul>\n<h3 class=\"bold_padding benefits\">Phase Two:<\/h3>\n<ul>\n<li>Facility\u2019s method(s) for monitoring ongoing education.<\/li>\n<li>Competency checklists for team members.<\/li>\n<li>Additional supplemental materials that assist with the review, such as publications, presentations, QA studies, data, and testimonials.<\/li>\n<li>Patient video(s) demonstrating the team\u2019s participation in the application of a <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span>, both on and off the ventilator (please use the Video Checklist to assist with requirements).<\/li>\n<\/ul>\n<h3 class=\"bold_padding benefits\">Phase Three:<\/h3>\n<ul>\n<li>On-site visit of the facility, which includes:<\/li>\n<ul>\n<li> Meeting with the team.<\/li>\n<li>Tour of the facility. <\/li>\n<li>Observation of the team with a patient.<\/li>\n<\/ul>\n<\/ul>\n<p>This application process occurs in three phases. With the successful completion of a phase, the facility will be asked to submit for the next phase. The Center of Excellence status is granted based upon exceptional execution of all the above criteria, and it continues year-to-year, as a facility maintains this high standard of care. <\/p>\n<p>Should you become a Center of Excellence, a part of your commitment is to facilitate ongoing multidisciplinary team management of your patients with tracheostomies. Your team must document that the members are continuing to participate in tracheostomy education and are familiar with the facility\u2019s <span class=\"pmv_ws\"><span class=\"pm_ws\"><span class=\"pm_ws\">Passy Muir<\/span><\/span> Valve<\/span> Policies and Procedures. <\/p>\n<p>In addition, <span class=\"pm_ws\">Passy Muir<\/span> conducts an annual review to confirm that the required, high quality, and multidisciplinary team management of patients with tracheostomy, necessary to be a Center of Excellence, have been maintained year-to-year. Passy-Muir conducts a Centers of Excellence recertification review every three years.
